BMI categories: <5% = Underweight, 5-84% = Recommended Range, 85-94%
= Overweight, >/=95% = Obese.
The SAS code for calculating BMI categories was obtained from the
National Center for Health Statistics' website entitled, "2000 CDC
Growth Charts: United States" (www.cdc.gov/growthcharts). The
calculations use the 2000 CDC growth charts, by AGE and SEX, as the
standard for assessing BMI status. Implausible values of height, weight
or BMI were excluded from the results, using World Health Organization
fixed exclusion ranges.
For some participants, callbacks were made to obtain more accurate
height and weight measurements (see "arrive at child's height/weight"
tables). The calculations of BMI-for-Age Categories include the
corrected heights & weights obtained from these callbacks for
children 10 and older.

**Health Insurance categories: PRIVATE =
State Employee Health Plan, Blue Cross/Blue Shield of NC, or other
private health insurance plan purchased from an employer or directly
from insurance company.
PUBLIC = Medicaid, Carolina ACCESS, Health Check, or NC Health Choice.
OTHER = the military, CHAMPUS, TRI CARE, Indian Health Servcies or
other type not otherwise listed.
