Cancer Survivorship

A cancer survivorship care plan is a detailed plan given to a patient that summarizes the patient’s cancer treatments, along with recommendations for follow-up care.
This plan may include schedules for physical exams and medical tests to check for health problems, including other types of cancer:
information to help meet the emotional, social, legal and financial needs of the patient:
as well as recommendations for changes in diet and exercise and quitting smoking.
Has any doctor, nurse, or other health professional ever given you a written cancer survivorship care plan at any point during or after your treatment of cancer?*
