SCHS: LDAS: Community Health Assessment
LDAS has responsibility for the review and approval of all community health assessments (CHAs), community health improvement plans (CHIPs), and state of the county health (SOTCH) reports submitted for local health department accreditation. The CHA-CHIP-SOTCH cycle occurs every three-four years.
In addition to requirements of the North Carolina Local Health Department Accreditation Board, DHHS/DPH provides supplemental guidance through its website, Fall and Spring CHA Institutes, and the Community Health Assessment Guidebook (PDF, 7.7 MB).
LDAS oversees the development and implementation of Healthy North Carolina 2030 (HNC 2030) and Healthy People 2030 (HP 2030).
